Print all pages and bring them to the Intake Interview.

Each Participant bring $75.00 for Intake Interview (cash, cashier’s check)

Bring “9” items to the
Intake Interview

Participants required to complete
intake interview:

Visiting Parent, Custodial Parent, other persons stated in the court order. 

Each person interviews separately.

Copy of Court Order or written instructions for supervised visitation.

Copies of Police Reports within the past 12 months, TPO Violence, Abuse, Drugs, Alcohol and Weapons.

Copies of Arrest or Conviction records within the past 12 months.

Copy of Driver’s License, Current Car Insurance Card, or Car Tag Number, Copy of Registration.

Completed Intake Form. Keep a copy for yourself.

Photos of child(ren)

Vaccine Card

Fee for Intake: $75 Cash or Cashier’s check % Georgia Preferred Family Resource (Each Participant)

Fee for the first visitation. Minimum of four hours for any supervised visitation. Advanced payment for supervised visitation is always required. Invoices for future visits are made through PayPal.
